An APCM Administrative Follow-Through
A primary care practice we support had an extensive APCM patient list that was becoming difficult to keep current. The work involved patient outreach, care plan updates, EHR documentation, and monthly tracking, alongside day-to-day patient care.
The practice had tried outside support before, but the list was not being maintained as expected. The issue was not simply assigning the task. The workflow needed consistent, day-to-day follow-through.
Now, someone is moving through the list steadily and keeping records aligned. Patient updates are captured, care plan follow-through is checked, and key information stays consistent between the EHR and the tracking spreadsheet.
Over time, that consistency makes the work easier to manage. Updates, documentation, and follow-ups are less likely to sit unfinished or scattered.
This is still early, so it is not a final outcomes report. The operational point is simpler: recurring work becomes more manageable when someone is consistently maintaining it.
