It happened a second time. Another referral, held while the team resolved an imaging requirement, closed early by someone who wasn't the owner.
No model hallucinated. No software crashed. Two patients fell through a gap that opened at a handoff, the moment a record passed between people whose roles overlapped but whose ownership was never defined.
This is what the AI conversation keeps missing. We argue about whether the model is accurate, whether the output is good. But most work doesn't fail at the output. It fails at the handoff: something gets generated, routed, and then dropped, because no one owned it through to completion.
The fix here wasn't technology. It was ownership, the referral team holds the record from initiation to close. Others can monitor, but monitoring is not the authority to end the work.
