Two recent audit cases we encountered.
An appointment appeared in the audit history with an agent's name on it.
There was no call recording. The agent told us they had no knowledge of it. Every other record we checked pointed somewhere else.
We could not establish who scheduled it.
The second case escalated to us. The patient had reported symptoms that warranted urgent evaluation. An appointment had reportedly been scheduled by a specific agent.
We went looking for a call recording, an encounter note, Teams correspondence, anything.
Nothing connected the named agent to the action.
We could not establish that either.
Here is the part worth sitting with.
Neither record came back blank. A blank field starts an investigation. A name ends one. In both cases the system produced a specific person, confidently, and every other source either contradicted it or said nothing.
Someone could have been coached on that record. Named in a complaint. Disciplined.
Neither case involved AI.
These were human workflows, where the evidence should be richest. We searched everything available and came back with nothing.
No outside auditor was involved either. These were our own reviews, run by people with full access to every system and every reason to find the answer.
What the audit history captured was that an action occurred. It did not capture that anyone decided anything. Those are different claims, and only one of them was ever written down.
Now consider what changes when software acts.
In both of these cases the evidence was supposed to exist. Someone was on a call. There should have been a recording. We searched because we had reason to believe something was there.
When software takes the action, nothing was ever supposed to be there. No call to recover. No agent to ask. The record is whatever the system wrote at the moment it acted, and that is the entire population of evidence anyone will ever have.
Both of these reviews took weeks and reached no conclusion, on single appointments.
Nobody runs that review at volume.
